The big departures from your run-of-the-mill fans are the angled frame behind the fan (which Noctua says better directs the airflow) and both the varied angular distances and vortex control notches that they say reduces noise. One thing I can say with certainty is that, on their own, these fans are absolutely silent. Operating via PWM, they can go as low as 300 RPM for even quieter operation. At full speed, all you hear is the whoosh of air with a hint of fan motor.
